Consent Form Notarizing
We will have a notary available in the Theater to notarize Medical and Liability Release Forms for students on Wednesday, February 14 from 7:50-8:20 pm. These forms are required for students who would like to participate in major upcoming events such as Disciple Now (DNow) or Super Summer Camp.
Forms will be available at this event or you can download a Medical and Liability Release Form here.
PLEASE NOTE: A parent or legal guardian must be present to sign the Medical and Liability Release Form. Pre-signed forms will not be notarized.
