Consent Form Notarizing
We will have a notary available in the Student Building to sign 2022 Ministry Consent Forms on May 4 from 7:40 – 8:30 pm.
The parent/guardian MUST be present to sign the form in the presence of the notary.
- Our church requires a parental consent form be completed for all students under the age of 18 for special student activities outside of regular Sunday or Wednesday events.
- If you have already completed a consent form this calendar year (2022), a new form is NOT required.
- If you have NOT completed a 2022 consent form, please download and print the attached form or pick up one from the church office.
- Drop off completed forms at the Church Office Monday-Friday from 8:00 am – 5:00 pm or bring on a Wednesday night to give to Pastor Jeromy.
