Kids & Students Consent Form Notarizing
We will have a notary available in the Theater to notarize Medical and Liability Release Forms for kids and students on Wednesday, January 21, from 7:45-8:15 pm. These forms are required for kids and students who would like to participate in major upcoming events such as the Students Hockey Game Outing, DNow (Disciple Now), or Summer Camp.
Forms are available in the Theater, or you can download a Medical and Liability Release Form here.
PLEASE NOTE: A parent or legal guardian must be present to sign the Medical and Liability Release Form. Pre-signed forms will not be notarized.
